This form is for the sole use for being reimbursed for expenses incurred as a result of getting involved in Tenant and Leaseholder Services, resident involvement activities.
You must submit this form within 2 calendar months of the date you incurred the expense(s).
If you haven’t yet provided your bank details please complete the bank details form first (this only needs to be completed once).
You will need all your relevant receipts to be able to complete this form. If you are only claiming mileage, no receipts will be required but you will need to know how many miles you have travelled.
Expenses are paid in line with our Resident Involvement Expenses and Incentive Policy which can be read on our Policies webpage.
