This form is for the sole use of being reimbursed for expenses incurred as a result of getting involved in Tenant and Leaseholder Services, resident involvement activities.

You must submit this form within 2 calendar months of the date you incurred the expense(s).

If you haven’t yet provided your bank details please complete the bank details form first (this only needs to be completed once).

Expenses are paid in line with our Resident Involvement Expenses and Incentive Policy which can be read on our Policies webpage.
