You will be asked for the following information so please have this ready before starting your request:

• Your name and address
• The type of restriction / change of restriction you are requesting
• Details of your request; exact location, length of restriction, times of restriction, boundary of restriction, etc
• Reason for your request, give as much details as possible and include in this section any support you may have from any other residents
• Any supporting evidence for your request; photos, of the issues, crash data, Emergency services access issue, etc
