Please complete this form if you wish to become a safe space in the community.

To become a safe space you will need to comply with the following:

• Hold a valid public liability insurance certificate
• Provide a safe and secure environment
• You must be able to provide a safe and secure environment
• There must be a minimum of 2 members of staff on duty at all times (where possible)
• Contact the appropriate agencies at the first opportunity if the person has been a victim of a crime or needs medical attention
• Contact the person named on the back of the card. If the person does not have a community shield card ask them to provide contact information for someone who can collect them e.g parent or carer
• Please ensure all of your staff are familiar with the community shield scheme and are able to provide support to those in need
